​I am in network with Aetna, Regence, Cigna, Carelon, Providence, Pacific Source, Kaiser Permanente NW, Moda, United, and Quest Behavioral Health. Additionally, I accept all major credit cards.  A "super bill" can be provided if I am not in network with your insurance provider. If paying out-of-pocket, initial assessments are $300 and follow up visits start at $150.

48 hours notice is required for cancellation so that I may better serve all of my patients. A $125 fee will be assessed for no shows and last minute cancellations.
